Vision and Hearing Screenings
Children in preschool, kindergarten, first, second, third, and special education will receive a hearing screening. Children in preschool, second, eighth, and special education will receive a vision screening. Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor. Your child is not required to undergo a vision screening if an optometrist or ophthalmologist completed and signed a report form indicating that an examination has been administered within the previous 12 months. If applicable please provide your school with a copy of your child’s eye examination. All screening is completed during regular school hours.

Vision/Hearing screenings will take place on these dates:

October 5 – Meadowview School
October 17-18 – Enrolled Pre-K Students at the Early Childhood Center
October 23 – Woodview School
October 26 – Frederick School
November 2-3 – Park Campus
November 13-14 – Avon Center School
November 15 – Grayslake Middle School
November 16 – Prairieview School

Exámen Visual y Auditivo
Los niños de preescolar, kindergarten, primero, segundo, tercero y educación especial recibirán un examen de audición. Los niños de preescolar, segundo, octavo y educación especial recibirán un examen de la vista. El examen de la vista no sustituye una evaluación completa de los ojos y la vista realizada por un oftalmólogo. Su hijo(a) no está obligado(a) a someterse a un examen de la vista si un optometrista u oftalmólogo completó y firmó un formulario de informe indicando que se ha administrado un examen dentro de los 12 meses anteriores. Si aplica, por favor proporcione a la escuela una copia del examen de la vista de su hijo(a). Todos los exámenes se realizan durante el horario escolar.

Los exámenes de la Vista/Audición se llevarán a cabo en las siguientes fechas:
5 de Octubre – Escuela Meadowview
17-18 de Octubre – Estudiantes de Pre-K inscritos en el Early Childhood Center
23 de Octubre – Escuela Woodview
26 de Octubre – Escuela Frederick
2-3 de Noviembre – Park Campus
13-14 de Noviembre – Avon Center School
15 de Noviembre – Grayslake Middle School
16 de Noviembre – Prairieview School
